Invoices will be sent within one week of receiving this registration form and your child's place will be confirmed upon receipt of payment.
Before the group starts, we would like to take this opportunity to remind you of the following conditions for enrollment in any of our summer group programmes:
1. Your child’s place is not confirmed until you pay the entire group fee. Once paid, registration fees are non-refundable, unless agreed to by Leith Stewart, Clinical Director.
2. Fees are required within two weeks of enrollment and/or the date of the invoice. Failure to pay will forfeit your child’s place on the programme.
3. Once a group programme has started, your child is not able to swap to another date or time.
4. If your child is late to the session, additional time will not be provided at the end of the session.
5. Unfortunately, there are no make-up classes, additional classes or reimbursement of session fee for missed group sessions, including if your child is unwell or for any other reason unless agreed to by Leith Stewart, Clinical Director.
6. There is no formal feedback meeting or progress report at the conclusion of the group programme. The therapist will be able to briefly talk to you about your child at the conclusion of the session/group programme if time permits.
7. The therapist will provide you with verbal recommendations and suggestions at the end of the camp.
8. If you do require a more formal individualised meeting or report, we would be happy to arrange a progress report and a 30 minute feedback meeting at the conclusion of the group. This will incur an additional cost of $1210.00
9. If the group is cancelled by the therapist or Young Folk Therapy, a group or individual session will be offered at a time that is suitable for both the therapist and the child and/or a reimbursement of the session fee.
10.Young Folk Therapy may allocate a different therapist to facilitate the group in the event of the original therapist's absence.
